담석증에서 약물과 식이

요약

담석증은 흔한 소화기질환 중 하나이며, 비만, 여성, 장기간의 금식, 빠른 체중감량, 총 경정맥영양 등 담석발생의 다양한 위험인자가 알려져 있다. 증상이 있는 담석의 치료는 담낭절제술이 일반적인 치료이다. 무증상 혹은 경한 증상을 보이는 담석증에서 담즙산제제인 UDCA가 담도산통의 유발이나 급성 담낭염, 급성 췌장염과 같은 합병증 발생을 낮춘다고 알려져 있으나, 대규모 전향적 연구가 필요한 실정이다. 담석, 특히 콜레스테롤 담석의 발생에 영향을 미치는 식이요소들로는 고콜레스테롤, 포화지방산, 정제된 탄수화물, 설탕 들이 있다. 예방적 효과를 보이는 식이는 불포화지방산, 섬유질, 커피, 적당양의 알콜 등이 있다. 또한 특정 음식에 대한 과민반응으로 담석 증상이 발혈 될 수 있으며, 그 외 영양 첨가물로 비타민 C, 레시틴, 철분 등이 영향을 미칠 수 있고, 식물 terpenes 복합물도 도움이 될 수 있다.

Abstract

Gallstone disease represents one of the most common gastroenterological disorders. Several risk factors for cholesterol gallstone formation in the general population have been identified. There is a strongly increased risk of gallstone disease during prolonged fasting, rapid weight loss, total parenteral nutrition, and somatostatin analogue treatment. Cholecystectomy is the most frequently recommended conventional treatment for symptomatic gallstones. In asymptomatic and symptomatic gallstone carriers, treatment with the hydrophilic bile salt ursodeoxycholic acid (UDCA) has been claimed to reduce the risk of biliary colic and gallstone complications such as acute cholecystitis and acute pancreatitis. However, randomized, double-blind, placebo-controlled trials are lacking. There is evidence that dietary factors influence the risk of developing cholesterol gallstones. Dietary factors that may increase risk include cholesterol, saturated fat, trans-fatty acids, refined sugar, and possibly legumes. Obesity is also a risk factor for gallstones. Dietary factors that may prevent the development of gallstones include polyunsaturated fat, monounsaturated fat, fiber, and caffeine. Consuming a vegetarian diet is also associated with decreased risk. In addition, identification and avoidance of allergenic foods frequently relieves symptoms of gallbladder disease, although it does not dissolve gallstones. Nutritional supplements that might help prevent gallstones include vitamin C, soy lecithin, and iron. In addition, a mixture of plant terpenes (Rowachol®) has been used with some success to dissolve radiolucent gallstones.

서 론

담석은 불용성 물질이 담낭 내에서 응결 혹은 부착되어 침전되는 복잡한 일련의 과정으로 형성되며, 동서양을 막론하고 드물지 않다. 담석을 지닌 대부분의 사람은 증상 없이 담석 동반을 모르고 지내는 경우도 많지만 일부에서 증상을 유발하여 담석증으로 나타난다. 담석증은 합병증 없이 담관산통만을 유발하기도 하지만 급성담낭염, 담관염, 담석성 췌장염 등 합병증을 야기할 수도 있다. 담석증에 있어 담낭은 담석의 발생 장소이고 원인도 될 수 있어 담낭 담석증의 치료는 담낭절제술은 가장 확실한 방법이다. 더욱이 대부분의 담낭절제술이 복강경 담낭절제술로 진행되는 요즘 담낭 담석증의 비수술적 치료의 역할은 크지 않다. 하지만 환자들이 담석증으로 호소하는 증상의 범위 및 강도가 매우 다양하고, 수술에 의한 담도손상 등 합병증 발생 가능성이 여전히 존재하며, 수술이 불가능하거나 거부하는 환자들이 있어 담낭 담석증 치료에 있어 비수술적 요법은 그 유용성이 여전하고 새롭게 주목 받을 필요가 있다. 이에 담석 및 담석증의 위험인자와 비수술적 치료의 선택, 또한 영향을 미치는 식, 생활습관과 약물, 식이에 대하여 소개하고자 한다.

본 론

1. 담석발생의 위험인자

담낭담석 특히 콜레스테롤 담석의 발생 위험인자로는 가족력, 고령, 여성, 임신, 출산경력, 여성호로몬치료, 비만, 급작스런 체중감량, 당뇨, 크론병, 척추손상, 장기간의 금식, 총 경정맥 영양, 옥트레오타이드(octreotide) 치료 등 다양하다. 또한 담석 혹은 담석증의 발생의 방어적인 약물로는 ursodeoxycholic acid (UDCA), statin, 비타민 C, ezetimibe, monoterpenes(Rowachol®) 등이 있고, 식이로는 불포화 지방산, 섬유질, 과일 및 야채, 커피, 중등도의 알코올섭취 등이 영향을 미치는 것으로 알려져 있다[1].

2. 담석증의 비수술적 치료의 선택

담석증 치료에 담낭절제술은 확실성, 비용효과, 안전 및 편의성을 고려하여 선호되며 복강경 담낭절제술의 도입 후 담석증의 비수술적 치료는 크게 감소하였다. 담석증의 비수술적 요법은 환자의 특성, 담낭의 기능, 담석의 크기, 수, 성분 등을 고려하여 선택하여야 한다. 수술이 불가한 심각한 내과적 문제가 있거나 수술을 거부하는 담낭염, 췌장염 등 합병증이 동반되지 않은 경한 혹은 중등도의 증상을 보이는 환자에서 내과적 치료를 고려할 수 있다. 특히 담석의 경구 용해 요법은 일부 선택적인 환자에서 수술적 치료를 대체하고 담석증의 발생위험이 높은 경우에서 예방적으로 사용될 수 있다.

3. 담석증의 예방

담석증의 예방은 일차, 이차, 삼차적 예방으로 나눌 수 있다. 일차적 예방은 담석의 발생위험이 높은 환자에서의 담석 발생의 예방, 이차적 예방은 담석을 지닌 환자에서 증상 발생의 예방, 삼차적 예방은 내과적 치료를 시행 받은 환자의 담석 재발의 예방을 의미한다[1,2].

1) 담석증의 일차적 예방(Primary prevention of gallstone)

가. 일반인의 일차적 담석 예방

이론적으로 적정체중의 유지, 일정한 간격의 식사, 고 섬유질 식이, 여성호로몬 복용을 피하는 것 등이 있다. 고 섬유질 식이의 섭취는 담석증 발생 빈도의 저하와 연관이 있고[3], 낮은 포화지방산의 섭취와 상대적으로 많은 불포화지방산의 섭취가 낮은 담석 유병률과 연관을 보고하였다[4,5]. 또한 지방산의 섭취와 관계없이 견과류의 소모가 담석증의 위험을 낮추고, 결과에 논란은 있지만 카페인이 포함된 커피[6,7], 논란은 있으나 알코올서 음료의 중등도 섭취도 증상이 있은 담석증을 예방하는 효과가 있다고 보고되고 있다[8,9]. 적절한 육체적 활동을 하는 남녀에서 낮은 육체적 활동을 보이는 군보다 담석증의 유병률이 낮았다[10,11].

나. 고위험군에서 일차적 담석 예방

빠른 체중감소와 장기간의 금식은 담석발생 고위험군으로 알려져 있다. 중등도의 체중감소(최대1.5 kg/wk) 이하를 유지하는 것이 담석발생률을 예방하고[12], 하루 10 g의 지방을 포함한 저칼로 섭취가 담낭배출을 도와 담석의 발생을 예방한다고 보고하였다[13]. UDCA의 복용은 체중감소중 담석증 예방에 효과적임을 보였다[14,15]. 총 정맥영양을 시행하는 환자에서는 매일 콜레시스토키닌(cholecystokinin, CKK)[16] 혹은 아미노산(amino acid)[17]를 정맥 주사하는 것이 담낭앙금(gallbladder sludge) 형성을 예방하는 것으로 보고하였다. 옥트레오타이드(octreotide)는 신경내분비종양 치료에 효과적이지만 식후 CCK 분비를 억제하는 역할로 담석형성의 위험을 증가시킨다[18]. 이러한 경우 함께 UDCA를 복용하는 것이 대부분에서 담석형성을 예방할 수 있다고 보고하였다[19].

2) 담석의 이차적 예방(Secondary prevention of gallstone)

가. 무증상 담석 혹은 담석증에서 담관산통 발생의 예방

무증상 담석의 담관성 통증 발생에 관한 연구는 드물지만 의미있는 과거 보고에서 대략적으로 매년 1%로 보고하고 있다[20]. 무증상 혹은 증상이 있는 담낭담석환자에서 하루 600mg의 UDCA를 투여하고 치료를 거절한 환자를 대조군으로 한 연구에서 UDCA를 복용한 환자들에서 담관선통의 발생빈도는 낮았지만 이 연구는 무작위 대조연구가 아닌 한계점이 있다[21]. 다른 연구들에서도 증상이 있었던 담낭담석 환자에서 UDCA복용 후 담관산통의 위험을 낮춘다는 보고들이 있었지만[22,23] 역시 무작위, 위약대조 실험이 아니었고, 담낭절제술이 예정되어 있는 증상이 있는 담석 환자군에서 시행된 대규모의 무작위, 이중맹검, 위약대조 연구에서는 UDCA는 담낭선통의 발생을 낮추는 효과를 입증하지 못하였다[24]. 그럼에도 불구하고 UDCA는 증상이 있는 담석환자에서 수술이 불가하거나 거부하는 경우 흔히 사용되고 있다.

나. 무증상 담석의 담석 합병증의 예방

급성담낭염이 가장 흔한 합병증이며, 급성 담석성 췌장염이 더 심한 임상경과를 보이는 경우가 많다. 무증상 담석증에서 UDCA의 복용이 급성 담낭염의 발생을 낮춘다고 보고 되었지만 역시 무작위 비교 연구들이 아니었다[21]. 급성 담석성 췌장염의 발생과 작은 담석 혹은 앙금, 담낭기능의 보존이 연관이 있다고 보고하였다[25].

다. 증상이 있는 담석증에서 합병증의 예방

담낭절제술은 증상이 있는 경우 합병증이 동반된 담석증으로의 진행을 막는 가장 중요한 치료이다. 조절이 가능한 담관 선통이 담낭염, 담관염, 췌장염, 폐쇄성 황달과 같은 합병증 동반없이 한달에 2-3회 미만으로 발생하는 중등도 이하의 증상에서 UDCA가 담석과 연관된 증상과 급성담낭염, 급성 담석성 췌장염과 같은 담석증 합병증의 위험을 낮춘다고 보고되었다[21,26,27]. 그러나 역시 무작위, 이중맹검, 위약대조연구에서는 증상이 있는 담석증 환자에서 담석 합병증에 대한 유용한 결과를 보이지 못했다[24]. 담관 선통를 보인 환자들에 대한 NSAIDs 치료가 급성담낭염의 발생위험을 낮추는 것으로 무작위 이중맹검 위약 비교 연구들을 통하여 보고되었다[28,29].

4. 담석증과 약물들(gallstone and drugs)

1) 담석증 위험을 증가시키는 약물들(risky drugs)

가. 에스트로젠 대체요법(estrogen replacement therapy)

에스트로젠은 중성지방 및 지방성분을 높여 콜레스테롤 담석의 위험을 높인다. 대부분의 연구에서 에스트로젠 치료는 높은 담석발생과 담석증에 의한 입원, 수술의 위험을 높이는 것으로 보고를 하고 있다[30-32]. 폐경여성에서 에스트론젠 단독과 에스트로젠 프로게스테론 병합치료를 시행한 군간의 무작위 비교 연구에서 에스트로겐 단독치료군에서 의미있게 높은 담낭 질환의 발생, 수술로의 진행이 있었다[32]. 대규모의 전향적 코호트 연구에서도 담석, 담낭염, 담낭절제술을 포함하는 담낭질환이 발생비율이 전혀 호르몬 치료를 받지 않은 사람에 비해 현재 그리고 과거 사용자에 비행 위험이 높았다. 그러나 에스트론젠 대체를 경구 복용이 아닌 패치나크림 형태의 시행한 경우는 위험이 증가하지 않았다[33].

나. Somatostatin analogue (Octreotide)

옥트레오타이드는 CCK 분비를 줄이고, 담낭배출을 억제하여 말단비대증이나 다른 질환의 치료를 위해 장기간 사용한 환자에서 담석의 위험이 증가한다[34,35]. 옥트레오타이치료를 시작한 첫 18개월에서 25%까지의 환자에서 증상이 없는 콜레스테롤 담석이나 담낭앙금의 발생이 보고되었다[35,36].

다. Clofibrate, other fibrates (gemfibrozol, fenofibrate)

담즙내로 분비되는 답즙산을 저하시켜 답즙내 cholesterol의 과포화를 유발하고 담석의 위험을 증가시킨다[37]. 다른 콜레스테롤 저하 약물은 담석증가의 위험은 없다.

2) 예방적 효과를 보이는 약물들(protective drugs)

가. Ursodeoxycholic acid (UDCA)

친수성 담즙산인 chenodeoxycholic acid (CDCA)와 ursodeoxycholic acid (UDCA)가 콜레스테롤 담석 경구용해요법치료에 사용되어 왔다. UDCA는 부작용이 거의 없고 설사도 덜 일으키며 혈중 cholesterol도 증가시키지 않으며 간독성도 없어 현재 가장 많이 사용되고 있으며 복합 제형도 사용되고 있다. 담즙산 제제는 담즙내 콜레스테롤 분비를 줄이고 담즙산 비율을 높여 결과적으로 콜레스테롤 포화도를 낮춘다. UDCA는 콜레스테롤의 담즙내 분비를 억제하고, 장내 콜레스테롤 흡수를 줄이고 담낭배출을 증가시킨다[38-41]. 담즙산 제제의 투여는 담석의 용해 뿐만아니라 증상의 경감과 합병증 발생의 빈도를 줄이기 위함이다.

나. Statin

Statin의 정기간의 사용은 담낭절제술이 필요한 담석증의 위험을 낮추는 것과 연관이 있다는 보고가 있으며[42,43] 그러나 보고에 따라 혼합된 결과를 보이고 있어 아직까지는 담석치료제로 statin을 권하는 것은 증거가 불충분 하다.

다. Ascorbic acid (Vitamin C)

동물 실험에서 비타민 C가 담석의 예방에 도움이 될 수 있다고 알려져 있으며 인간을 대상으로한 단면연구에서 혈청 vitamin C level과 담석증이 빈도에 역 상관관계를 보고하고 있다[44].

라. Ezetimibe

장내 콜레스테롤 흡수 억제제로 혈중 콜레스테를 낮춘다. 사람을 대상으로 한 연구에서는 콜레스테롤 포화지수를 낮추고 콜레스테롤이 결정화되는 것을 늦춰는 것을 보여주었다[45]. 그렇지만 사람에서 담석예방과 용해에 효과적인지 여부에 대해서는 추후 더 많은 연구가 필요하다.

마. Terpenes 혼합물(Rowachol®)

식물성 terpenes 혼합물 Rowacol®은 간내 콜레스테롤 합성을 낮추고 담즙내 콜레스테롤 용해도를 높혀 콜레스테롤 담석의 용해를 높고 추가적 형석을 줄일 수 있다[46,47]. 이 제형은 화학적 특성 상 단단한 물질의 용해 및 붕괴의 효과가 있어 방사성 투과성 담석과 일부 방사선 비투과성 담석을 용해와 담도 산통의 경감을 시킬 수 있으며 UDCA와 병합했을 때 효과가 강화된다고 보고하고 있다[48].

5. 담석과 식이요소들(gallstone and dietary factors)

담석 특히 콜레스테롤 담석의 발생에 영향을 미치는 식이요소들은 다양한데, 위험을 높일 수 있는 요소들은 콜레스테롤, 포화지방, 트랜스 지방산, 정제된 설탕 등이고 비만도 담석 발생의 위험인자이다. 담석 발생을 예방할 수 있는 요소들로는 불포화지방, 섬유질, 카페인, 채식 등이 있다. 특정 음식에 의해 유발되는 알레르기성 담석증에서는 유발음식을 피하는 것도 담석을 용해하지는 못하지만 증상의 발생을 피할 수는 있다. 그러나 알레르기성 담석증의 유발음식은 개인에 따라 모두 다를 수 있어 유발 음식을 찾아내고 피하는 것이 중요하다. 그러므로 담석증 환자에서 일반적으로 기름진 지방식이나 과식을 피하는 식이의 권고만으로는 항상 만족할만한 결과를 얻지는 못할 수 있다. 그 외 연구적 증명은 약하지만 영양보조제로 비타민 C, 레시틴, 철분 등이 담석 예방에 도움이 될 수 있다[49].

가. 담낭에 도움이 되는 음식들

신선한 과일과 야채, 견과류(nuts; 땅콩, 아몬드, 호두 등), 미정재 곡물(통밀, 현미, 귀리, 밀기울 등), 살코기, 생선, 가금류, 저지방 음식, 카페인 포함 커피, 중등도의 알코올섭취 등.

나. 담낭질환에 피해야 할 음식들

정제된 탄수화물, 흰 밀가루로 만든 음식, 설탕, 감미료 포함 음식, 음료수, 스낵(파이, 감자칩 등), 고지방식이, 튀김음식, 지방이 많은 붉은고기, 전유를 가공한 유제품(치즈, 아이스크림 등), 아주 낮은 칼로섭취(1,000 kcal/day 이하)

결 론

담석증은 일반 인구에서 적절한 칼로리 섭취를 통한 이상적인 체중을 유지하고, 섬유질을 많이 섭취하고 적은 포화지방산을 소비 하고 중등도의 신체 활동을 하는 등의 생활 및 식이섭취를 통하여 그 위험을 낮출 수 있다. 하지만 이러한 권장 식생활은 기타 다른 심장질환, 당뇨, 고혈압 등을 예방하기 위한 권고와 크게 다르지 않다. 담석 및 담석증 발생의 위험이 동반된 경우는 UDCA의 복용으로 담도 산통의 위험, 합병증의 발생을 감소시킨다고 보고되었으나 대규모 전향적 연구에서는 UDCA의 효과는 입증되지 못했다. 총 경정맥 영양 환자에서는 매일 정맥내 CCK 혹은 아미노산 주사가 담낭앙금 형성을 방지할 수 있다. 담도 산통의 NSAIDs치료는 급성 담낭염의 위험을 줄일 수 있다. 특정 영양 보조제들은 담석을 방지하는 데 도움이 될 수 있지만 연구의 근거가 강하지 못하다.
