Abstract

췌장염 환자의 10–30%는 그 원인을 찾지 못하여 특발성 췌장염으로 분류되며, 그중 일부는 유전적 변이가 원인일 것으로 생각된다. 유전성 췌장염은 소아 때부터 췌장염 관련 증상이 발생하여 대부분 30대 전후로 만성 췌장염으로 진행하기 때문에 췌장암의 발생에 대해 각별한 주의가 필요하다. 췌장염 관련 유전자는 현재까지 30종류 이상 발견되었으며, 대표적으로 PRSS1, SPINK1, CFTR, CTRC 변이가 있으며, 이는 인종 및 지역에 따라 다양성을 보이고 있다. 따라서, 국내 특발성 췌장염 환자를 대상으로 한 유전자 연구를 통하여 한국인에서의 췌장염 관련 유전자 변이의 양상 및 특징을 파악하는 것이 중요하겠다. 본고에서는 특발성 췌장염의 병태생리에서 유전적 변이의 역할에 대해 중점적으로 고찰해보고, 국내 문헌을 토대로 특발성 혹은 유전성 췌장염의 국내 연구 결과를 살펴보고자 한다.

Trans Abstract

10–30% of patients with pancreatitis can be categorized as idiopathic pancreatitis, and some of them may be due to genetic alterations. Since hereditary pancreatitis develops from pediatric patients with symptoms related to pancreatitis, which usually progresses to chronic pancreatitis around 30 years of age, special attention should be paid to the development of pancreatic cancer in such patients. Up to now, there have been more than 30 genetic alterations associated with pancreatitis. Alterations in protease serine 1 (PRSS1), serine protease inhibitor Kazal type 1 (SPINK1), cystic fibrosis transmembrane conductance regulator (CFTR) and chymotrypsin C (CTRC) are common, which show diversity according to race and region. It is important to understand the characteristics of Korean patients with idiopathic pancreatitis through genetic studies. The purpose of this article is to review the role of genetic variations in the pathophysiology of idiopathic pancreatitis and to survey the results of Korean studies of idiopathic pancreatitis.

서 론

췌장염은 다양한 원인에 의하여 췌장의 염증 및 이로 인한 전신적 손상이 발생하는 질환으로, 반복적인 췌장염은 만성 췌장염으로 진행하면서 결국 비가역적 췌장 실질 파괴 및 섬유성 변화가 일어나 췌장의 외분비 및 내분비 기능의 저하를 초래하게 된다[1]. 급성 췌장염의 주 원인은 알코올 남용 및 담석으로 알려져 있으나, 그 외 약제, 쇼크, 외상, 감염, 대사성 질환, 해부학적 변이 등의 원인이 있다[2]. 그러나 10-30%의 췌장염 환자에서는 그 원인을 찾지 못하는데, 이 경우를 특발성 췌장염으로 정의하며, 이 환자들은 원인이 불분명하여 치료가 쉽지 않다[3]. 과거에는 특발성 췌장염의 재발률이 높지 않아 원인을 파악하기 위한 검사를 최소한으로 할 것을 권고하였으나, 최근의 연구에서는 췌장염이 50% 이상에서 재발하는 것으로 보고되어, 원인을 찾고 재발을 방지하기 위한 노력이 필요하다. 또한, 특발성 췌장염은 향후 만성 췌장염으로 진행할 가능성이 높으며, 이 경우 췌장암의 위험이 상승하며, 사망률 또한 다른 원인의 췌장염에 비하여 높은 것으로 알려져 있다[4]. 특발성 췌장염 환자 중 일부는 유전적 변이가 관련 있을 것으로 보고되고 있어[5], 본고에서는 특발성 췌장염의 병태생리에서 유전적 변이의 역할에 대해 중점적으로 고찰해보고, 국내 문헌을 토대로 특발성 췌장염의 국내 연구 결과를 살펴보고자 한다.

유전성 췌장염

유전성 췌장염은 동일 가계 내에 2세대 이상에 걸쳐서 3명 이상의 췌장염 환자가 있으면서, 상염색체 우성 유전을 나타내며 질환 투과도(penetrance)는 약 80%이다[6,7]. 유전성 췌장염은 질환의 발생이 대부분 10세 이하의 소아 때 발생하고, 가장 흔한 증상은 췌성 통증(80%)이며 약 70%에서 급성 췌장염이 관찰되는 반면 중증 급성 췌장염의 발생은 드문 것으로 알려져 있다[7]. 프랑스에서 시행된 유전성 췌장염의 대규모 코호트 연구에서 평균 22–25세경 췌장 석회화 등 만성 췌장염 소견을 보이며, 외분비 기능 장애는 평균 29세에 34%에서, 내분비 기능 장애는 평균 38세에 26%에서 발생하였다[8]. 따라서, 유전성 췌장염 환자에서는 만성 췌장염의 유병 기간이 길어지기 때문에 췌장암의 위험도 증가하여 50대에 이르면 일반인에 비하여 췌장암 위험도가 약 53배 상승한다[9]. 연령에 따른 췌장암 누적 위험도는 각각 50세에 10%, 60세에 18.7% 그리고 75세에 53.5%인 것으로 보고되었다[8]. 따라서 유전성 췌장염 환자에서는 췌장암 선별 검사가 필요하나 아직까지 표준 선별 검사가 확립되지 않은 실정이다. 일반적으로 40세 이상부터 췌장암 선별 검사를 권하고 있으며, 초음파 내시경이 유전성 췌장염에서 pancreatic intraepithelial neoplasia (PanIN) 변화를 식별하는데 도움이 된다고 하나, 만성 췌장염 변화가 심한 경우에는 진단율이 떨어지게 된다[7]. 그 외 K-ras 유전자 변이, 단백체학(proteomics) 등이 선별 검사에 도움이 될 것으로 기대된다[7,10].

유전성 혹은 특발성 췌장염과 관련된 유전자 변이는 현재까지 30종류 이상 발견되었으며, 대표적으로 serine protease 1 (PRSS1), serine protease inhibitor Kazal type 1 (SPINK1), cystic fibrosis transmembrane conductance regulator (CFTR), chymotrypsin C (CTRC), calcium-sensing receptor (CASR), carboxypeptidase A1 (CPA1) 변이가 알려져 있다(Table 1) [11-18]. 그러나 유전성 췌장염 가계 중 25–67%에서는 원인이 되는 유전적 돌연변이를 찾지 못하고 있다[19]. 초기에 국내 보고에서 한국에서는 특발성 만성 췌장염에서 PRSS1 및 SPINK1 변이가 매우 드물어 특발성 췌장염의 원인으로서 비중이 작다고 밝혔다[14,20]. 그러나 2005년에 가성 낭종 및 췌관 출혈을 동반한 만성 췌장염 소아 환자, 어머니 및 여동생에게서 PRSS1의 R122H 변이가 국내 최초로 보고된 이후[21], 국내에서도 PRSS1, SPINK1, CFTR 변이가 드물지 않으며, 흥미롭게도 서양에 비하여 SPINK1 IVS3+2T>C 변이가 흔한 것으로 밝혀졌다[22-24].

Genetic alterations in patients with idiopathic or hereditary pancreatitis

PRSS1 변이

PRSS1은 특발성 췌장염의 발생에 관여하는 유전적 변이로 처음 밝혀진 유전자이며, T세포 수용체 beta locus내 염색체 7q35에 위치하고 있는데 이 부위에서 DNA sequencing을 하다가 우연히 8개의 trypsinogen 유전자를 발견한 것이다[11,25]. 이 locus에는 PRSS1 외에도 5개의 trypsinogen pseudogene과 anionic trypsinogen을 코딩하는 PRSS2를 포함하고 있다[26]. 상염색체 우성 유전성 췌장염 가계에서 cationic trypsinogen을 생성하는 PRSS1 122번째 아미노산인 arginine (CGC)이 histidine (CAC)으로 변하는 exon 3의 p.R122H (c.365G>A)변이가 1996년에 Whitcomb에 의하여 처음 발견되었고[11], 1997–1998년에 걸쳐 두 번째로 흔한 29번째 아미노산인 asparagine (AAC)이 isoleucine (ATC)으로 변하는 exon 2의 p.N29I (c.86A>T) 변이가 발견되었다. 이 두 가지 이형접합체 변이가 전세계적으로 유전성 췌장염에서 90% 가까이 발견되고 있으며[26], 그 외 exon 1에서 -28 delTCC, exon 2에서 A16V, D22G, K23R, P36R, exon 3에서 E79K, G83E, K92N, V123M 등의 다른 변이도 발견되었다[6,19]. PRSS1 변이는 trypsinogen 자가활성화 및 CTRC-dependent trypsinogen 분해 억제를 통하여 단백 분해 효소와 항단백 분해 효소 간에 불균형을 초래시키고, trypsin의 췌장 내 활성도를 증가시킨다[27]. 다른 PRSS1 변이인 p.R116C에서는 trypsinogen misfolding이 발생하여 trypsin의 세포내 축적 및 분해를 유발시키고, 결국 분비를 감소시킨다[28]. 그러나 모든 PRSS1 변이가 병적인 것은 아니며, 드문 변이의 임상 관련성을 예측하기 어려울 수 있다.

SPINK1 변이

SPINK1은 1948년에 Kazal 등[29]에 의해 trypsin inhibitor로 발견되었으며, 염색체 5번에 위치하는 4개의 exon으로 이루어진 7.5 kb 유전자이다[30]. SPINK1 변이는 단독으로 직접 췌장염을 일으키는 원인으로 작용하기보다는 알코올 등 다른 췌장염의 원인에 의한 췌장염 발생의 역치를 낮추거나, CFTR 등 다른 유전자와 상호 작용을 하여 췌장염의 위험도를 높이는 것으로 알려져 있다[31]. SPINK1 변이는 흔한 편으로 전체 인구의 0.5–2.5%에서 발생하는데, 특발성 췌장염 환자에서는 16–23% 정도로 높다고 보고되며, 이 중 N34S (c.101A>G) 유전자 변이가 가장 흔하게 발견된다[5,32]. N34S 변이는 흥미롭게도 두 종류의 인트론 변이, IVS1-37T>C 및 IVS3-69insTTTT와 관련된 것으로 보이며, 이는 N34S 자체보다는 인트론 변이가 췌장염 발생에 연관이 있는 것으로 보인다[32-34]. 그러나 이 인트론 변이는 어떤 비정상 접합도 일으키지 않아 SPINK1 유전자의 mRNA 발현에 영향을 끼치지 않기 때문에, SPINK1 변이는 trypsinogen의 autophagy 유발을 통하여 췌장염을 일으킨다는 가설도 제시되고 있다[32].

CFTR 변이

CFTR은 서구에 비교적 흔한 유전 질환인 낭성섬유증(cystic fibrosis)과 관련 있는 것으로 알려진 유전자로, 7q31에 위치하며 췌관 세포에 발현하여 췌액 분비를 담당하는 것으로 알려져 있다[17,35]. 췌관 세포에서 bicarbonate의 분비는 CFTR을 통하여 이루어지며, SLC26A3 및 SLC26A6 등의 다른 막 수송체는 bicarbonate가 분비되는 동안 억제되기 때문에 CFTR이 bicarbonate 이동의 핵심이다[36]. 또한, CFTR 변이가 발생하면 apical membrane chloride channel에 이상이 발생하여 비정상적으로 췌액의 점도가 높아져 췌관에서 췌액 분비에 장애가 발생한다[37]. 따라서, CFTR 관련 췌장염은 췌장 acinar cell보다는 췌관 세포에서 기인하며, 장기간 췌액 분비 장애로 인한 췌관내 압력 상승으로 이차적으로 만성적 췌관 변화를 야기하며 만성 췌장염으로 진행하게 된다[38,39]. 췌장염과 연관된 CFTR의 가장 흔한 변이는 F508Del과 R75Q로 알려져 있다[39].

CTRC 변이

CTRC는 trypsin 분해 효소와 관련된 것으로 알려져 있어 trypsin으로부터 췌장을 보호해 줄 것으로 기대되며[40], 유럽 췌장염 환자 코호트를 통하여 CTRC에서 두 종류의 변이, p.R254W 및 p.K247_R254del이 알코올성 췌장염, 열대성 췌장염 환자에서 대조군보다 유의하게 더 많이 발견되었다[41]. 그러나, 이후 다른 연구에서는 CTRC 변이와 췌장염의 직접적 인과관계를 밝히는데 실패하여, 아직까지 논란의 여지가 있는 바, 이에 대하여 대규모 환자-대조군 연구가 필요한 실정이다[39,42].

국내 특발성 췌장염의 유전적 변이 연구 현황

특발성 췌장염의 발생률 및 관련 유전자 변이는 지역 및 인종에 따라 다양한 양상을 보이며, PRSS1 변이를 포함하여 현재까지 알려진 췌장염 관련 유전자 변이에 대한 연구는 주로 유럽 및 미국에서 백인을 대상으로 시행되었다[2]. 미국 캘리포니아 지역의 급성 췌장염 환자 전체를 대상으로 한 역학 조사에서 전체 급성 췌장염과 특발성 췌장염의 발생률이 모두 흑인에서 가장 높았고, 다음으로 히스패닉계, 백인, 아시아인의 순서를 보였으며, 아시아인의 경우 인구 10만 명당 특발성 췌장염의 연간 발생률이 11.2명으로 가장 낮았다[43]. 그러나 미국 전역을 대상으로 하였던 다른 연구에서 전체 췌장염 중 특발성 췌장염의 비율이 백인에서 37%, 유색인종에서 35.4%로 유사한 양상으로 보였다[44]. 2007년 일본에서 시행된 연구에서 급성 췌장염 중 특발성 췌장염의 비율은 16.7%로 나타났다[19]. PRSS1 변이의 빈도도 인종에 따라 다른 것으로 알려져 있는데, 북미는 52%, 유럽 전역은 81%, 프랑스는 68%, 일본은 44%, 인도는 0%의 유전성 췌장염 환자에서 PRSS1 변이가 관찰되었다[45]. 앞에서 언급한 바와 같이 초기에 국내에서는 특발성 만성 췌장염의 유전적 변이가 매우 드문 것으로 알려졌으나[14,20], 이후 Oh 등[22]의 국내 특발성 혹은 가족성 췌장염 환자 연구에서 PRSS1 변이가 13%의 환자에서 발견되었고 이 중 가장 흔한 아미노산 변이는 R122H였다. 다른 국내 연구들에서 PRSS1, SPINK1, CFTR 변이가 유전성 혹은 특발성 췌장염의 발병에 역할을 하는 것으로 밝혀졌다[22-24]. 그러나 아직까지 특발성 췌장염의 유전적 원인에 대한 국내 연구는 부족한 실정으로 소아 환자에게 발생하는 유전성 췌장염에 대한 증례 보고 수준의 연구가 있었다[21]. 이에 대한췌담도학회에서는 2016년에 특발성의 만성 또는 급성 재발성 췌장염 환자를 대상으로 whole exome sequencing을 이용한 다기관 연구를 진행하였고, 2017년 현재 결과 분석 중에 있다. 특발성 췌장염 관련 유전자 변이가 많이 발견됨에 따라 특이 exome을 스크리닝하고, sequencing하는 데 시간과 비용이 많이 들게 된다. 그러나 최근 각광받고 있는 next generation sequencing을 이용하여 대량의 parallel sequencing이 가능해짐에 따라 시간 및 비용을 절약할 수 있다[46,47]. 향후 이 연구 결과를 토대로 국내 특발성 췌장염 환자들의 특성을 파악하고, 특발성 혹은 유전성 췌장염 환자의 선별 검사 및 치료에 도움을 줄 수 있을 것으로 기대된다.

결 론

특발성 췌장염에서 유전적 변이의 역할에 대한 연구는 약 20여 년 간의 짧은 기간 동안 이루어졌고, 국내 연구는 거의 없는 실정이지만, 최근 next generation sequencing 등 인간 유전자 정보 전체를 대량으로 신속하게 분석할 수 있는 방법의 개발이 가속화됨에 따라 급속할 발전이 이루어질 것이라 기대된다. 향후 이를 통하여 췌장염의 병태생리에 대해 근본적인 이해와 함께 특발성 췌장염의 진단 및 치료에 도움이 될 수 있겠다.
