담관 및 췌관 결석 치료에서 홀뮴레이저 쇄석술: 신의료기술 등재에 따른 임상 및 제도적 고찰

담관 및 췌관 결석은 일반적으로 내시경역행담췌관조영술(endoscopic retrograde cholangiopancreatography, ERCP)을 통해 제거가 가능하다[1]. 그러나 결석이 크거나 위치가 복잡한 경우, 또는 기존의 기계적 쇄석술로 충분한 분쇄가 어려운 경우에는 완전 제거에 한계가 있다. 이러한 난치성 결석에서 홀뮴레이저(Holmium laser)를 이용한 쇄석술은 효과적인 대안적 치료법으로 주목받고 있다[2].

최근 단일 시술자 담도내시경(single operator cholangioscopy, SOC)를 이용하면 결석을 직접 시야에서 확인하며 쇄석과 제거가 가능해져 시술의 정밀도와 안전성이 향상되었다[3]. 이러한 기술적 발전과 축적된 임상 근거를 바탕으로, 2024년 5월 30일 한국보건의료연구원은 보건복지부 신의료기술평가위원회를 통해 홀뮴레이저를 이용한 경구 또는 경피경간 내시경적 담관 및 췌관 결석 제거술의 안전성과 유효성을 인정하여 신의료기술로 최종 승인하였다(보건복지부 고시 제2024-96호).

한편, 2025년 11월 11일자로 의료기기 분류 및 사용 목적에 ‘레이저 체내충격파 쇄석기’ 항목이 추가되면서, 홀뮴레이저 기반 쇄석 장비의 제도적 정의가 보다 명확해졌다. 현재까지는 국내에서 판매되는 홀뮴레이저 장비 중 사용목적에 결석 쇄석과 관련된 사항이 명시된 제품은 Lumenis PulseTM 30H (Boston Scientific, Marlborough, MA, USA)가 유일하며, 이는 담관 및 췌관 결석 쇄석에 대한 제도적 근거를 갖춘 장비로 평가된다. 이러한 규제 환경의 정비는 임상 현장에서 홀뮴레이저 쇄석술의 적절한 적용과 표준화에 중요한 기반을 제공한다.

시술 과정 및 기전

경구 담도내시경을 통해 담관이나 췌관으로 접근하여 레이저로 결석을 분쇄한 후 파쇄된 결석 조각을 제거할 수 있다. 경피경간 접근법은 간내 담관에 위치한 복잡한 결석에도 접근이 가능하여 ERCP로 접근이 어려운 경우 효과적인 대안이 될 수 있다(Fig. 1). 이러한 접근법은 특히 해부학적 구조가 복잡하거나, 이전 수술 이력으로 인해 내시경 접근이 제한된 환자에서 유용하다.

홀뮴 레이저는 약 2.1 μm 파장의 에너지를 이용하며, 이 파장은 결석을 둘러싼 수분 환경과 결석 내부의 수분에 의해 효율적으로 흡수된다. 흡수된 레이저 에너지는 국소적인 열 효과를 유발하여 결석 표면에 미세 균열을 형성하고, 동시에 수분의 급격한 기화로 인한 미세 기포의 생성과 붕괴를 반복적으로 유도한다. 이러한 과정에서 발생하는 순간적인 압력 파동은 결석 분쇄를 촉진하며, 광열적(photothermal) 작용과 광기계적(photomechanical) 작용의 결합을 통해 효과적인 결석 파쇄가 이루어진다. 이 기전은 주변 연부 조직으로의 에너지 확산을 제한하여 조직 손상을 최소화하는 장점이 있다.

현재 임상에서 사용되는 홀뮴레이저 쇄석 시스템은 짧고 정밀한 펄스형 에너지를 안정적으로 전달하도록 설계되어 있으며, 다양한 직경의 광섬유와 호환되어 담관 및 췌관내시경 시술에 적용 가능하다. 이러한 기술적 특성은 결석에 대한 효율적인 에너지 전달과 함께 시술의 정밀도와 안전성을 향상시키는 데 기여한다.

임상적 유효성

여러 연구에서 홀뮴레이저 쇄석술은 높은 결석 제거 성공률을 보였다. Bang 등[4]은 SOC를 이용한 쇄석술에서 단일 세션 기준 93.9%의 완전 결석 제거율을 보고하였고, Angsuwatcharakon 등[5]의 연구에서도 방사선 노출을 줄이면서 높은 성공률이 확인되었다. Alexandrino 등[6]의 다기관 연구는 결석의 크기와 개수가 시술 성공률에 영향을 미치는 요인으로 나타났으며, 특히 직경 22 mm 이하의 결석에서 단일 세션 완전 제거 가능성이 높았다. Rimon 등[7]은 경피경간 접근법을 통해 대부분의 환자에서 완전 결석 제거에 성공하였고, 장기 추적 관찰에서도 재발 없이 안정적인 경과를 보고하였다. Roux-en-Y 간공장문합술 병력이 있는 환자에서도 경피적 접근법의 유효성과 안전성이 보고되었다[8].

안전성

홀뮴레이저 쇄석술은 전반적으로 안전성이 우수한 것으로 평가된다. 보고된 합병증은 주로 경미한 췌장염이나 낮은 비율의 담관염에 국한되었으며, 중대한 시술 관련 합병증은 드물었다[7,9]. 실제 임상에서는 전기수압쇄석술(electrohydraulic lithotripsy, EHL)이 오랜 기간 널리 사용되어 온 표준적 쇄석 기법으로, 장비 접근성이 용이하고 대부분의 담관 결석에서 효과적인 장점을 가진다. 기존 연구에서 홀뮴레이저 쇄석술과 EHL 간의 전반적인 안전성은 유사한 수준으로 보고되었으나, 홀뮴레이저 쇄석술은 결석에 대한 정밀한 에너지 전달과 수분 선택적 흡수 특성으로 인해 주변 담관 점막 손상을 상대적으로 최소화할 수 있어, 특히 복잡하거나 단단한 결석에서 임상적 이점을 가질 수 있다[10,11]. 이러한 결과들은 기존 치료에 실패한 환자에서도 홀뮴레이저 쇄석술이 안전하고 효과적인 대안이 될 수 있음을 시사한다.

췌관 결석 치료에서의 응용

홀뮴레이저 쇄석술은 췌관 결석 치료에도 적용 가능하다. 기존 기계적 쇄석술로 제거가 어려운 경화된 대형 결석에서, 경구췌관내시경(peroral pancreatoscopy)을 이용한 레이저 쇄석술은 높은 결석 제거율을 보였다. Han 등[12]은 크거나 다수의 췌관 결석에서 레이저 쇄석술이 효과적임을 보고하였다. 또한, 다기관 연구에서 EHL 또는 레이저 쇄석술을 받은 환자들의 췌관 결석 제거율은 89.5%로 높았으며, 레이저 쇄석술은 EHL에 비해 평균 시술 시간이 유의하게 짧았다 (53.8분 vs. 74.4분, p<0.001) [13].

결 론

홀뮴레이저 쇄석술은 기존 내시경적 방법으로 치료가 어려운 담관 및 췌관 결석 환자에서 높은 성공률과 우수한 안전성을 보이는 최소침습적 치료법이다. 신의료기술 등재와 더불어 레이저 체내충격파 쇄석기 사용목적의 제도적 명확화는 임상 적용의 근거를 강화하며, 향후 복잡 담관 및 췌관 결석 치료에서의 활용 범위 확대에 기여할 것으로 기대된다.
